Nov. 15

M-U Bulldog Diner, 30 Lowry Dr., West Milton — Observed employee food in non-designated area on top of stainless surface top. Observed wet wiping cloths sitting on table in back section near prep cooler. Critical violation: observed ice machine with residual build-up in top and bottom of machine. Observed bottom of freezer with residual oil build-up. Observed damaged floor next to mop sink. Observed ice in walk-in cooler. Observed a build-up of residual under pop dispensing machine in cooler.

Nov. 16

Papa John’s, 779 W. Market St., Troy — Critical violation: person-in-charge did not ensure that employees are informed in a verifiable manner of their responsibility to report information about their health. Person-in-charge did not ensure that the facility has written procedures for employees to follow when responding to vomiting or diarrheal events. Observed iPad and speaker sitting on the corner of pizza prep cooler next to ingredients. Observed handles of pizza prep cooler with residual food build-up. Observed employee coat hanging on storage rack coming into contact with clean cutting boards and other equipment. Observed missing tile near water heater in back area. Observed residual build-up and debris on floor under prep sink.

Subway, 1281 S. Dorset Rd., Troy — Observed employee drink being stored on prep table near clean items. Critical violation: person-in-charge did not ensure that employees are informed in a verifiable manner of their responsibility to report information about their health. Person-in-charge did not ensure that the facility has written procedures for employees to follow when responding to vomiting or diarrheal events. Observed gasket on walk-in freezer with black build-up that was coming loose from the door. Observed residual build-up on handles of equipment throughout the facility and behind the ice chute on the pop machine in dining area. Observed a build-up of debris and food residual underneath shelving in storage area.

Subway, 982 W. Main St., Tipp City — Critical violation: person-in-charge did not ensure that employees are informed in a verifiable manner of their responsibility to report information about their health. Person-in-charge did not ensure that the facility has written procedures for employees to follow when responding to vomiting or diarrheal events. Observed residual build-up on handles of cabinets behind the sandwich prep line and on handles of the microwave. Observed debris and food residual under shelves of the walk-in cooler and walk-in freezer, and on the floor behind the prep sink.

VFW Pleasant Hill, 7578 Fenner Rd., Pleasant Hill — Critical violation: person-in-charge did not ensure that employees are informed in a verifiable manner of their responsibility to report information about their health. Observed food in packaging that does not appear to protect it from contamination. Observed multiple dented cans inside walk-in cooler.Observed improper storage of food items. Juice in upright cooler was observed to be stored without being covered up. Critical violation: refrigerated, read-to-eat, TCS foods held refrigerated for more than 24 hours that were not properly date marked. Observed chili being held in a single serve item in walk-in cooler past its original date mark. Observed residential cooker being used to cold-hold chili in upright cooler. Repeat observation: observed large amounts of standing water on the bottom of beer cooler behind bar counter. Observed re-use of single-use items. Critical violation: ice machine is dirty and has an accumulation of mold and residual on the top of the machine and where ice is coming from. Observed debris and food residual inside of door track of upright cooler. Observed food residual on the front of the microwave, especially the door opener button. Observed no towels or hand drying device at the hand washing sink.

Steak N Shake, 1779 W. Main St., Troy — Observed ice cream in walk-in cooler being stored without having ice cream lid on. Observed large accumulation underneath drainage tubing behind fans in walk-in cooler. Critical violation: observed food residual build-up on the top of the inside of microwave. Observed food residue on upright cooler gaskets, on the handle of the upright cooler, all over kitchen floor, and on floor of the walk-in freezer.

Nov. 17

The Brewery, 111 W. Water St., Troy — Walk-in cooler toward the back of the facility was not in good repair and leaking. Observed water pooling up in the bottom of the beer cooler behind bar counter, Critical violation: observed mold build-up in drink nozzle behind counter. Observed mold on the top part of the inside of the ice machine. Popcorn machine needs to be cleaned on the inside walls. Observed no hand washing signage at sinks used by employees.

Smith’s Boathouse Restaurant, 439 N. Elm St., Troy — Observed employee drinks in kitchen prep area and not in a designated area. Critical violation: observed food employee touching ready-to-eat food with bare hands. Observed employee placing lemon slices on plate with other ready-to-eat foods without wearing any gloves. Observed ice scoop being stored on top of the ice machine. Observed wet wiping clothes not stored in sanitizing bucket. Observed gloves being stored on top of hand washing facility behind bar counter. Critical violation: observed working container of cleaner that was not properly labeled with the name of the material.

Nov. 18

Hobart Arena southeast concessions, 255 Adams St., Troy — Observed good hand washing and glove usage. Employees all have level 1 training. No violations at time of inspection.

Cassano’s, 975 W. Main St., Tipp City — Critical violation: food employees did not wash hands in situations that specifically require them to do so. Observed person-in-charge using cell phone and not washing hands afterwards. Observed employee drink being stored on top of prep table. Observed bowls being stored in prep cooler, touching food. Equipment is not approved by a recognized testing agency. Observed dust build-up on the fan above the hot holding unit. Observed parts of the wall above the three compartment sink with rough caulking and holes in the wall. Observed that the ventilation system was not being properly cleaned. Critical violation: observed working containers of poisonous or toxic chemicals that were not properly labeled with the names of the material. Corrected during inspection.

Cassano’s, 1201 Experiment Farm Rd., Troy — Observed employee drink being stored in walk-in cooler outside of the designated area for employee items. Observed bowls being stored in top portion of the pizza prep unit, touching food. Observed employee clothing and personal items being stored on racks in dry stock area. Critical violation: observed working container of over cleaner under three compartment sink that was not properly labeled. Corrected during inspection. Observed good glove usage along with frequent hand washing. Temperature logs were kept. Person-in-charge was very knowledgeable about food safety.
