Dad’s Carry Out, 249 Union St., Troy — Person-in-charge did not ensure that the FSO or RFE have written procedures for employees to follow when responding to vomiting or diarrheal events. Cold or hot holding RTE foods did not have a thermometer inside of the unit. Non-food contact surfaces of equipment are dirty. Observed dust build-up in fan casing on roof of cold holding unit RTE foods. Observed debris such as gravel and dirt on the floor inside of the shed outside behind store.
Arby’s, 903 W. Main St., Troy — Did not have a person-in-charge that had completed a level on certification course. Observed improper storage of food items. Observed soda syrup bag laying directly on the ground towards the back of the storage room connecting to soda machine. Observed improper storage of personal food items on top rack of walk-in cooler. Non-food contact surfaces of equipment are dirty. Observed food residue build-up on the back of handles, on walk-in cooler handle, and ToastMaster handles. Cleaned equipment and utensils, laundered linens, or single-service and single use articles are not properly stored. Napkins were stored in open carboard boxes directly on the floor in storage area. Observed broken tiles beneath register near front of ordering table. Observed a build-up of food debris on the floor of the walk-in cooler, on floor in the break room and the back room. Observed a build-up of debris in the drain underneath the prep sink towards the back of the kitche, Observed mild along caulking at the top of the three-compartment sink.
Burger King, 1829 W. Main St., Troy — Observed very little to no grout on tiled floor throughout kitchen area. Observed a residual build-up and debris under rubber mats and on floor of walk-in cooler. Observed a build-up of hand-contact residual on and behind the handle of the walk-in cooler. Observed a build-up of oil residual and debris within the mop sink. Observed a build-up of residual and debris along floor and walls near the water heater, behind and around the ice machine, and a build-up of grease and food behind grill and fryer area.
China Garden Buffet, 15 S. Weston Rd., Troy — Critical violation: observed raw chicken stored over green beans in walk-in cooler. Food was not properly protected from contamination by separation, packaging, and segregation. Corrected during inspection. Observed measuring cup being stored in enclosed flour bins with handle immersed in food product. In-use utensils are improperly stored. Observed food items in boxes stored on floor of walk-in freezer. Critical violation: observed bulk chicken being kept out of refrigeration. Corrected during inspection. Observed sushi out in buffet where time as a public health control was being used without the proper written procedures. Critical violation: observed dented cans in dry stock. Observed a Pepsi cooler meant for storing bottled beverages and packaged foods storing time/temperature controlled for safety foods. Equipment is not approved by a recognized testing agency. Observed condensation issue with condenser in walk-in freezer. Observed cold-holding buffet where the bottom of the cabinet flooring was water damaged, cracking, and contained mold. Walk-in freezer gaskets not intact, tight, or properly adjusted. Observed food residual and build-up on and behind handle of walk-in cooler, along base of faucet in the dish tank area, and in microwave. Observed shelving racks over dish tank area with build-up and debris. Observed a cracked and broken faucet at the hand sink and a water leak on faucet of three-compartment sink. Observed outside storage area near grease container with black oil residual. Observed an improperly cleaned storage area for refuse, recyclables, or returnables. Observed light bulbs that were not properly shielded or coated over buffet tables. Observed no supply of hand-cleaning liquid, powder, or bar soap at the hand-washing sink next to the ice machine in server station. Observed broken and loose tiles throughout facility. Critical violation: observed the presence of insects, rodents, and other pests. Observed the presence of dead roaches in cabinet under the ice cream table.
Dominos Pizza, 937 W. Main St., Troy — Critical violation: person-in-charge did not ensure that employees are informed in a verifiable manner of their responsibility to report their health information. Prep cooler equipment did not contain a thermometer or the thermometer was not correctly placed. Observed improper display or storage of poisonous or toxic materials above three-compartment sink.
Fayevores Banquet Center, 2334 S. Miami St., West Milton — Observed improper storage of food items. Drinks were found to be sitting in boxes on the ground of the walk-in cooler. Observed dust build-up on fan case inside of walk-in cooler. Observed food residual in cold-holding unit.
Ording’s Party Time, 810 S. Market St., Troy — Critical violation: observed food that was not properly protected from contamination by separation, packaging, and segregation. Meat needs to be covered while being stored to prevent contamination. The cheesecake was disposed. Critical violation: refrigerated, RTE, TCS foods held refrigerated for more than 24 hours were not properly date marked. Items disposed. Critical violation: food surfaces were not easily cleanable or not designed or constructed properly. A saw used to cut meat was found to be rusty.
Ruby Tuesday, 1780 W. Main St., Troy — Critical violation: TCS foods were not being held at the proper temperature. Corrected during inspection. Observed water leak at faucet underneath bar area counter and at three-compartment sink. Observed doors for upright cooler right behind cold holding unit were not staying shut. Observed damage to wall insert toward the right hand side in the ice machine. Surfaces of cutting blocks or boards were severely scratched or scored and could not be effectively sanitized. Temperature gauge on mechanical dishwasher was not reaching the appropriate temperature. Observed clogged drain underneath beer tap from excess yeast.
He5Asian Bistro, 121 E. Staunton Rd., Troy — Observed employee drinks in non-designated area near food prep. Critical violation: observed open raw meat in walk-in cooler stored over vegetables. Observed food that was not properly protected from contamination. Observed sauces that were made in the restaurant sitting out on cook line that are considered a temperate controlled for safety food. A thermometer capable of accurately measuring thin foods was not available. Observed meat grinder attachment for large mixer not cleaned properly. Observed coats hanging on shelves. Critical violation: observed the presence of rodent droppings in storage areas.
Pizza Hut, 1480 W. Main St., Troy — Observed residual build-up on and behind handles of walk-in cooler and freezer.
The Cakery, 1875 W. Main St., Troy — Observed employee drinks being kept on tables next to prepped food. Critical violation: person-in-charge did not ensure that employees are informed in a verifiable manner of their responsibility to report their health information. Observed wet wiping cloths that were not being kept in sanitizer. Observed food residual in microwave and on upper mixer components. Critical violation: observed cell phone sitting on top of clean muffin tin.
